Dear Families and Youth/Young Adults with Disabilities:

SPAN is pleased to provide you with this Family Guide to Transition
Services in New Jersey. By providing one cohesive document on the
transition process, services, and supports, we hope to make the journey
easier for everyone. Transition can be a complicated time—this guide
can answer your questions and can make transition a successful period
for families and youth/young adults with disabilities.

SPAN would like to thank the Massachusetts Federation For Children
with Special Needs for their contribution to the development of this
Transition Guide. SPAN regularly collaborates with all the federally-
funded Parent Centers in the Northeast US to continue our effort to
positively impact families and youth/young adult with disabilities.

SPAN’s vision is that all families in New Jersey will have the resources
and support they need to ensure that their children become fully
participating and contributing members of our community and society.
